To get started we will need to request a credit card release form to allow SMA to make auto payments on the 1st of each month. This program will automatically renew each year at 5% increase due to product cost increase that we occur each year. You will be notified 60 days in advance a reminder of your renewal and new draft amount. A $30 return fee will be assessed for any transaction not processed.
